Excellent plan choices are available for employees and their eligible dependents. Current options include a PPO, HMO, and two HSA PPO plans. The plans include prescription drug benefits, as well as wellness programs and incentives. Each plan provides excellent coverage through Aetna’s nationwide network with more than 1,000,000 health care professionals. Annual open enrollment allows ease of transfer between plans.
You and your eligible dependents have the opportunity to enroll in dental plans through Aetna. The Dental PPO (two plans – “Value PPO” and “Premium PPO”) offers the flexibility of seeing a dentist in network and receiving benefits at a discounted rate, or seeing a non-network dentist and paying the difference between the reasonable and customary charges and what your dentist bills. Under the Dental DMO, there are no deductibles and most dental benefits are unlimited.
You may elect vision coverage through Vision Service Plan (VSP) for your eyewear and eye care needs. VSP consists of over 22,400 member doctor locations throughout the United States. The plan offers up to $130 annually toward new frames, plus an additional 20% off out-of-pocket costs. Lenses are covered at 100% (of plan allowance) in-network, and exams are a $20 co-pay.
The 401(k) plan is an excellent opportunity for eligible employees to save for retirement. Whether participating in a traditional or Roth plan, after eligibility is met employees have the opportunity to contribute up to the annual IRS maximum amount. The plan also allows for easy rollover of funds, and a loan option.
An HSA allows an employee to set aside a portion of his or her earnings (pre-tax) into an account to pay for qualified medical expenses. The Company will contribute (bi-weekly) $500 for employee only, and $1000 for employee plus dependent(s) coverage. Contributions can be made to your HSA up to the limits set by the U.S. Treasury and the IRS. Unused balances roll over year-to-year.
Because we are concerned about the well-being of you and your family, the Company funds 100% of the cost of a Basic Life and AD&D policy for all eligible employees through Hartford. The policy will pay a benefit equal to two times your annual compensation (rounded to the nearest $1,000), up to a maximum of $500,000. As an eligible employee, you are automatically covered by Basic Life and AD&D insurance; you do not have to enroll.
Short and Long Term Disability benefits are provided through Hartford. This is a company paid benefit at no additional cost to the employee.
Hartford’s Employee Assistance Program, administered by ComPsych, puts convenient and confidential resources within reach to help you and your loved ones with life’s challenges.
iMatrix allocates 15 days paid time to each employee per year who have passed their 90 day training period. These days are allocated between sick and personal days at the employee’s discretion. iMatrix also has 8 paid Holidays that are available upon initial hire date.
Internet Matrix, Inc., is affiliated with Bank of America. Direct Deposit of your paycheck will automatically remove monthly account fees.
